PATIENT INFO/FORMS

Prior to starting therapy, you can get started following these three easy steps:

​

Step 1:  Schedule an Appointment

Please call our office at (509)884-1437 to schedule your first appointment. You will be asked what type of injury you have and how recent your injury occurred. This will help us determine your need for specific treatment. Please be sure to have your insurance information ready at the time of scheduling your appointment.

Step 2:  Fill Out Patient Forms 

Prior to your first visit, we ask that you download the New Patient Registration Packet and bring the completed forms with your insurance card to your first appointment.

 

In addition, please select the form for the body area that focuses on your chief complaint and fill it out. You may select more than one form if you are being seen for more than one body part.

Step 3:  Arrive For Your Appointment

Please plan on arriving to the clinic at least 10 minutes prior to your scheduled appointment time. This allows you time to check in and sign any other forms that may be needed. If you plan on filling out all forms including the patient registration packet and questionnaires in the clinic, please arrive at least 20 minutes prior to your scheduled appointment time. 

 

Please be sure to bring the following items with you to your first appointment:

  • The completed forms listed above.

  • Your current insurance card(s).

  • A method of payment (i.e., checkbook, credit card, cash.).

  • Comfortable clothing as noted below:

    • For low back pain or lower extremity problems, please bring shorts and a t-shirt.

    • For females with back, neck, and/or upper extremity problems, please bring shorts and a halter-top.  Note that a sport back jog bra obscures viewing of the shoulder blades and is not recommended
